Due
to a recent update by Adobe, if you have downloaded
the most recent version of Adobe Reader (Adobe
Reader X) you'll be unable to save your information
in the document. If you're still running Adobe
Reader 7, Adobe Reader 8, Adobe Reader 9, or Adobe
Reader 9.4, you will still be able to save without
issue. Please be advised we are working on a
sollution and in the meantime you can still place
your application over the phone or by printing the
